Coaches’ Player Rating Form
At the end of each season, a player rating form must be completed for every player. Coaches must complete the form and deliver to the Age Group Coordinator upon completion of each season. Coaches are urged to complete these forms quickly and completely, as a coaches' fair assessment is critical to ensuring proper placement and development of players. A Player rating form need to be completed at the end of each season for each player on their team.

Coaches Team Roster forms
Coaches and assistant coaches for U9 through U14 teams must have an official team roster provided by the Registrar. Coaches for U5 through U8 teams will receive a list of players from their Age Group Coordinator.
